Noise Pollution And Your Heart

The Nobel Prize Winner Robert Koch predicted in 1910 that ‘One day man will have to fight noise as fiercely as cholera and pest’.

I frequently read and reply to articles on Quora, and find that a number of people worry about the noise levels around them. 

Well, they should!!

Noise pollution is a rampant problem in India. With millions of vehicles on the roads and the constant honking for no reason whatsoever, the excessive amount of noise has become a nuisance. There is a common misconception that noise pollution only affects the ears. Recent evidence suggests that noise pollution can affect your heart. Up until now, this was an unknown fact.

Researchers from the Johannes Gutenberg University in Germany took at a look at all the studies that had evaluated the role of noise in the development of heart disease. What they found was quite alarming.



It appears that noise can affect the heart at a cellular level. The high levels of noise can induce changes in the levels of certain stress hormones and nerve functions. This stress leads to the damage of blood vessels. Not just that, the excessive noise can induce what is called ‘oxidative stress’. This leads to the formation of harmful free radicals that can damage the heart muscle and other vital tissues. Noise can also lead to disturbed sleep.

Living in a noisy neighborhood where traffic is constantly moving can lead to poor sleep which in turn can increase blood pressure. High blood pressure is a risk factor for heart disease. Experts suggest that constant exposure to sound over 60 decibels can cause heart damage. To give you some perspective, the sound with which we speak at home or in an office is around 60 dB. The sound of a jackhammer is around 100 dB.

With this knowledge, it is important that the government takes steps to lower noise pollution. Honking should be banned if possible. Noise cancelling headphones may be useful if you are exposed to loud noises all the time. However, these can be quite expensive.

Testing For Heart Blockages - Step By Step Guide. Dr Vivek Baliga Writes.

One of the most common questions I get asked as a doctor is whether or not a person has ‘heart blockages’.

The term is a very broad one and many a time it is unclear to us as to what the patient is trying to convey. From what I gather, patients wish to know if the blood supply to the heart muscle is reduced and whether they are at a risk of a heart attack.

For the purpose of this article, I will talk about heart blockages in the same context as patients refer to it. This way, I hope to give you a clear understanding as to how we can detect this problem through both non-invasive and invasive methods.

Testing For Heart Blockages

In the first instance, the most important thing for us as doctors is to know what the history of the patient is.

Is there a history of chest pain? 

Is this chest pain any worse when walking and is it relieved upon resting? 

Is the pain any worse when you move or take a deep breath in? 

The answer to these questions can help give us a better idea as to whether or not the pain an
individual is experiencing is heart -related.

If the symptoms are suggestive of heart disease, the first test to perform is an electrocardiogram, or ECG.

An electrocardiogram is useful in detecting whether there are any electrical changes within the heart muscle that have been brought upon by narrowing in the heart arteries.

Please note that it is quite normal for patients to have a completely normal electrocardiogram but still have heart artery disease.

However, if an ECG is conducted at the same time a patient is experiencing chest pain, a normal ECG usually rules out disease. That being said, by looking at the risk factor profile, we
decide whether or not further investigation is required.

The next best test to learn about the health of the heart is an echocardiogram, also called an ‘echo test’.

This is an ultrasound test of the heart that can help find out how well the heart is pumping and how efficient it is. It also looks at whether or not any damage has occurred to the heart muscle due to reduce circulation to a particular part. It can also find out whether there are any holes in the heart or problems with any of the heart valves.

An echocardiogram can sometimes show subtle changes which are indicative of reduced circulation to the heart muscle. I personally have picked up hidden heart disease in many patients just by doing an echocardiogram.

Given that it is a painless test, it is essential that it is performed as a part of a cardiac evaluation.

If the echocardiogram is unremarkable, the next test would be a treadmill test.

The treadmill test is one that will help determine whether the heart is receiving adequate amount of blood during times of stress. By walking on a treadmill at a particular speed, the patient is put under a deal of cardiac stress which should unmask problems with circulation through either changes on the ECG or the occurrence of symptoms such as chest pain.

Once again, it is important to note that a negative treadmill test does not necessarily rule out heart disease completely. In fact, one in every five treadmill tests can be negative despite there being some degree of heart artery narrowing.

Given that it is a non-invasive test, it becomes less sensitive than the invasive tests described below.

Depending on how positive the treadmill test is, further investigations such as an angiogram may be performed. An angiogram can be performed either as a CT scan or as a conventional angiogram in the hospital.

The former is reserved for patients who are at a low to intermediate risk (fewer risk factors) while the latter is reserved for more high-risk patients.

An angiogram is the best test to determine if there are heart blockages. These days they are performed as day procedures and do not require admission to hospital as such.

Do I Need All These Tests?

Not everybody requires all these investigations. Depending on the risk factor profile such as a history of smoking, family history of heart disease, abnormally high cholesterol levels, the presence of high blood pressure and diabetes, a determination will be made as to what test would be appropriate.

The best way to know if you have heart blockages is to speak to your doctor and get yourself tested.

There are no specific guidelines when it comes to what tests are needed, but there are some that your doctor may recommend.



A haemoglobin level is important, as a low level of haemoglobin can lead to breathlessness during physical exertion. An electrocardiogram will help determine if there is any problem with the heart. Walking

The resistance that the water provides can increase the heart rate and strengthen your heart as well. Studies have found that exercises like swimming can lower cholesterol levels and burn calories at a remarkable pace.

Cycling

You don't have to hit the road on the bicycle if you wish to keep your blood sugar is low. This could be dangerous in Indian traffic!

Instead, you could purchase a stationary bike or use the one in your local gymnasium. Riding a bike for about 30 minutes a day at a reasonable level of resistance can power up your heart and lower your blood sugar levels. It also is light on the knees and does not stress out your joints. Try and cycle at least five days a week to keep your diabetes under control.

Climbing stairs

A common aspect of maintaining good health that we often forget is climbing stairs. While it can be quite an effort and sometimes even very tiring, the calories that are burnt in climbing stairs are remarkable.

Not just that, it also strengthens your heart and your lungs. If you wish to lower your blood sugars and keep the diabetes under control, try and walk up and down stairs for just 3 minutes 1 – 2 hours after you have had a meal. Whenever possible, take the stairs and burn calories, not
electricity.

Lightweights 

When strength training i.e. lifting lightweights is combined with aerobic exercises, the benefits you reap are increased manifold. Make sure you combine your walking or other aerobic exercise routines with strengthening of your muscles and bones through light weight lifting. You could do this about 2 to 3 times a week. Simple exercises such as push-ups, squats and pull-ups are sufficient to keep the muscles and the bones strong.

Gardening

Gardening involves different movements such as walking, bending and kneeling, all of which can keep the blood flowing and the blood sugar burning. It can also help lower stress and is a great exercise you don't like traditional activities.

Hair loss is a common problem in men. Hormones, ageing and many other factors play a role. Hair loss in women seems to be less common, but I do see many women in the clinic complaining of this problem.

The growth of hair occurs in 3 cycles – anagen, catagen and telogen. 90% of the hair on our heads is in the anagen phase, or growth phase. The rest are in the shrinking (catagen) or resting (telogen) phase. On an average, we all lose around 50 to 100 strands of hair every day.

We could lose up to 250 hair strands when washing our hair (not washing it does not change this – it will fall out eventually!).

So why do women lose hair? 

Hair loss can be a genetic problem in most cases. It usually affects the front of the head, making the forehead a lot more prominent. Close observation of the hair follicles using a magnifying glass shows both thin and thick follicles. This is a common cause of hair loss and is called androgenetic alopecia. It is a genetic condition that runs in the families.

So how can women realise that they are losing hair? 

What is normal and what is not? In men, hair loss tends to occur from the forehead, moving in a backward fashion. They can also lose hair around the crown, which is the spot on the back of the head.

In women, hair loss begins with thinning of hair around the top third of the scalp. The gap between
hair follicles increases gradually, and areas of the scalp begin to become visible. Normally, when hair loss occurs, the lost hair is replaced by a hair that is of the same size. When hair loss occurs in women, the new hair that grows is thinner and finer. In other words, the hair follicles begin to shrink and ultimately stop growing completely.

Conditions such as pregnancy, low hemoglobin levels, polycystic ovarian syndrome and thyroid disease can lead to hair loss. It is therefore essential to investigate these.

Treating hair loss is simple. Avoid hairstyles that involve pulling the hair back. The use of chemicals, blow dryers and flat irons can damage hair.

Toweling the hair aggressively when drying it can also cause hair loss. You may want to consider hair fall supplements for women.

The simple measures can reduce the loss of hair. Medical therapies are also available that can help these days.
